By using our website or voluntarily providing information to us, you acknowledge that you have read this Privacy Policy.

1. WHO WE ARE

Praxis Integrated Healthcare LLC is a U.S.-organized healthcare consulting and integrated solutions company.

Praxis provides services that may include healthcare consulting, digital health solutions, healthcare procurement, and non-clinical international patient coordination and facilitation.

Praxis is not, solely by virtue of offering these services, a hospital, clinic, health plan, medical practice, or healthcare provider.

For questions regarding this Privacy Policy or our privacy practices, please contact us using the information in the Contact Us section.

2. SCOPE OF THIS PRIVACY POLICY

This Privacy Policy applies to personal information we obtain through:

  • the Praxis website;

  • website contact forms;

  • requests for information about our services;

  • email communications;

  • prospective client inquiries;

  • International Patient Services inquiries;

  • consulting inquiries;

  • procurement inquiries;

  • vendor and supplier communications;

  • partnership inquiries;

  • website analytics;

  • cookies and similar technologies;

  • newsletter or marketing subscriptions, if offered; and

  • other interactions that link to this Privacy Policy.

This Privacy Policy may not govern information processed under a separate written agreement, data processing agreement, Business Associate Agreement, confidentiality agreement, healthcare-provider privacy notice, or other specific contractual or legal arrangement.

Where another legally applicable agreement or privacy notice governs particular information, that document may supplement or control over this Privacy Policy with respect to that information.

3. PERSONAL INFORMATION WE MAY COLLECT

The information we collect depends on how you interact with Praxis.

A. Information You Provide Directly

We may collect information you voluntarily provide, such as:

  • name;

  • email address;

  • telephone number;

  • country or general location;

  • company or organization name;

  • professional title;

  • preferred method of communication;

  • service of interest;

  • information contained in messages or correspondence;

  • project or business requirements;

  • procurement requirements;

  • scheduling information;

  • and other information you choose to provide.

4. INTERNATIONAL PATIENT SERVICES INFORMATION

Praxis International Patient Services is intended to provide non-clinical coordination, facilitation, navigation, and related support for individuals seeking healthcare services from independent healthcare providers.

Praxis does not generally request, collect, or maintain detailed medical records through its website.

When you contact us regarding International Patient Services, we may collect limited information reasonably necessary to understand and coordinate your request, such as:

  • your name;

  • contact information;

  • country of residence;

  • preferred healthcare destination;

  • the general medical specialty or type of healthcare service sought;

  • preferred timing;

  • scheduling preferences;

  • language or communication preferences;

  • and relevant non-clinical logistical information.

For example, an individual may voluntarily tell us that they are seeking oncology, cardiology, orthopedic, surgical, or other specialized care.

Such a statement may reveal limited health-related information. Praxis may receive and use that information to understand the nature of the requested assistance and facilitate an appropriate connection.

Medical Records and Detailed Clinical Information

Praxis does not generally require patients to provide us with:

  • complete medical records;

  • laboratory results;

  • imaging files;

  • pathology reports;

  • detailed medical histories;

  • physician notes;

  • prescriptions;

  • treatment plans;

  • genetic information;

  • or other extensive clinical documentation.

If a hospital, clinic, physician, or other healthcare provider requires medical records or clinical information to evaluate a potential patient, Praxis will generally direct the individual to submit that information directly to the relevant healthcare provider or through a secure system designated by that provider.

We encourage individuals not to send detailed medical records or highly sensitive medical documents through the Praxis website or general email channels unless Praxis has specifically instructed otherwise.

5. VOLUNTARILY DISCLOSED HEALTH INFORMATION

Individuals sometimes voluntarily disclose limited health-related information while describing the services they are seeking.

For example, a person may tell Praxis:

  • that they have been diagnosed with a particular condition;

  • that they are seeking treatment for a particular illness;

  • that they need a particular medical specialty;

  • or that they are seeking a second opinion or procedure.

Praxis may process such voluntarily provided information only to the extent reasonably necessary to respond to the inquiry, understand the requested service, facilitate communication, or coordinate requested non-clinical assistance.

Praxis does not intend to use voluntarily provided health information to independently:

  • diagnose a medical condition;

  • determine medical necessity;

  • recommend a clinical treatment plan;

  • prescribe treatment;

  • interpret diagnostic studies;

  • provide a medical second opinion;

  • or substitute its judgment for that of a licensed healthcare professional.

Clinical evaluation and medical decision-making remain the responsibility of the independent healthcare provider involved.

6. INFORMATION WE DO NOT INTENTIONALLY SEEK THROUGH GENERAL WEBSITE FORMS

Unless specifically required for a legitimate business purpose and an appropriate collection method is provided, please do not submit the following through general website forms:

  • complete medical records;

  • imaging files;

  • laboratory reports;

  • pathology reports;

  • Social Security numbers;

  • national identification numbers;

  • passport scans;

  • passwords;

  • credit or debit card credentials;

  • bank account credentials;

  • or other highly sensitive information.

If such information becomes necessary for a particular service, Praxis may provide separate instructions regarding how and where it should be submitted.

7. BUSINESS AND PROFESSIONAL INFORMATION

For Healthcare Consulting, Digital Health Solutions, Healthcare Procurement, partnerships, vendors, suppliers, and other professional relationships, we may collect information such as:

  • business contact information;

  • professional title;

  • employer or organization;

  • organizational needs;

  • project descriptions;

  • procurement specifications;

  • requests for proposals or quotations;

  • supplier information;

  • product information;

  • correspondence;

  • contract-related information;

  • and other business information relevant to a proposed or existing engagement.

8. INFORMATION COLLECTED AUTOMATICALLY

When you visit our website, certain information may be collected automatically by our website platform, hosting provider, analytics providers, security systems, or similar technologies.

This information may include:

  • browser type;

  • device type;

  • operating system;

  • approximate location derived from IP address;

  • referring website;

  • pages viewed;

  • links clicked;

  • date and time of access;

  • website interaction information;

  • session identifiers;

  • cookie identifiers;

  • and technical information used for security, analytics, diagnostics, and website functionality.

10. LEGAL BASES FOR PROCESSING

Where applicable law requires us to identify a legal basis for processing, Praxis may rely on one or more of the following:

Consent

We may process personal information where you voluntarily provide consent.

Where applicable law permits withdrawal of consent, you may withdraw it at any time. Withdrawal does not affect processing lawfully conducted before withdrawal.

Contractual Necessity

We may process information where necessary to:

  • take steps requested before entering into an agreement;

  • enter into an agreement with you;

  • or perform contractual obligations.

Legitimate Interests

We may process information where reasonably necessary for legitimate business purposes, including:

  • and protecting legal rights.

Legal Obligations

We may process information where necessary to comply with applicable laws, regulations, court orders, or other binding legal obligations.

Sensitive Information

Where applicable law provides additional protections for health-related or other sensitive information, we will rely on an appropriate legal condition when such a condition is required.

11. HOW WE MAY DISCLOSE INFORMATION

Praxis does not sell personal information for monetary compensation.

We may disclose personal information where reasonably necessary for legitimate service, operational, contractual, or legal purposes.

Healthcare Providers

When an individual requests International Patient Services, we may share limited information with an independent:

  • hospital;

  • clinic;

  • healthcare system;

  • physician practice;

  • treatment center;

  • or other healthcare organization

where reasonably necessary to facilitate a requested introduction, inquiry, appointment, or administrative coordination.

Where detailed medical records are required, patients will generally be instructed to provide those records directly to the healthcare provider.

Healthcare providers independently determine how they collect, use, maintain, and protect patient medical information under their own privacy and legal obligations.

Technology and Service Providers

We may use third parties that support:

  • website hosting;

  • cloud infrastructure;

  • email;

  • business communications;

  • cybersecurity;

  • analytics;

  • scheduling;

  • customer relationship management;

  • document management;

  • accounting;

  • or other business operations.

These providers may process information as necessary to provide their services.

Suppliers and Business Partners

For procurement or commercial activities, relevant business information may be shared with:

  • suppliers;

  • manufacturers;

  • distributors;

  • logistics providers;

  • consultants;

  • and other business partners involved in a proposed transaction.

Professional Advisers

We may share information where appropriate with:

  • lawyers;

  • accountants;

  • auditors;

  • insurers;

  • compliance professionals;

  • and other advisers.

Legal Authorities

We may disclose information if reasonably necessary to:

  • comply with applicable law;

  • respond to lawful legal process;

  • comply with court orders;

  • respond to regulatory or governmental requests;

  • prevent fraud or unlawful activity;

  • or protect the rights, property, security, or safety of Praxis or others.

Corporate Transactions

Information may be disclosed in connection with a proposed or completed:

  • merger;

  • acquisition;

  • financing;

  • restructuring;

  • sale of assets;

  • reorganization;

  • or similar business transaction.

12. INDEPENDENT HEALTHCARE PROVIDERS

Healthcare providers introduced or identified through Praxis are independent organizations or professionals.

Their collection and handling of medical information is governed by their own:

  • privacy policies;

  • notices of privacy practices;

  • security procedures;

  • contractual obligations;

  • and applicable healthcare and privacy laws.

Unless expressly agreed otherwise in writing, Praxis does not control an independent healthcare provider's clinical records system or privacy practices.

Patients should review the relevant healthcare provider's privacy notices before submitting medical information to that provider.

13. HIPAA AND HEALTH INFORMATION

The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its implementing regulations apply to particular categories of organizations, including certain healthcare providers, health plans, healthcare clearinghouses, and business associates.

Praxis's involvement in the healthcare industry does not by itself make Praxis a HIPAA-covered entity.

Similarly, the voluntary disclosure of health-related information by an individual directly to Praxis does not, by itself, establish that the information is Protected Health Information (“PHI”) governed by HIPAA in Praxis's possession.

Praxis generally structures International Patient Services so that detailed medical records and clinical documentation are transmitted directly between the individual and the relevant healthcare provider rather than routinely collected or maintained by Praxis.

In some circumstances, however, Praxis may enter into a relationship with a HIPAA-covered organization under which Praxis performs services involving PHI.

If Praxis is acting as a “business associate” as defined under HIPAA in connection with a particular engagement, Praxis will comply with the HIPAA requirements applicable to that role and any applicable Business Associate Agreement.

Nothing in this Privacy Policy should be interpreted as:

  • representing that all information held by Praxis constitutes PHI;

  • representing that Praxis is a HIPAA-covered healthcare provider;

  • or expanding HIPAA beyond its legal scope.

Nothing in this Privacy Policy replaces a healthcare provider's legally required Notice of Privacy Practices.

14. OTHER HEALTH-PRIVACY LAWS

Health-related information may be protected by laws other than HIPAA.

Even where HIPAA does not apply, Praxis may be subject to other federal, state, national, or international privacy, consumer-protection, data-security, or health-information requirements depending on the circumstances.

Praxis seeks to collect only the information reasonably necessary for the relevant service and to use it only for appropriate business purposes.

15. WEBSITE INFRASTRUCTURE

The Praxis website may be hosted or operated through Wix and related technology infrastructure.

Wix and its service providers may process certain website information in connection with:

  • hosting;

  • website functionality;

  • cybersecurity;

  • content delivery;

  • visitor sessions;

  • cookies;

  • website forms;

  • analytics;

  • and related technical services.

The information processed through Wix may be subject to Wix's applicable privacy practices and contractual terms in addition to this Privacy Policy.

Praxis does not control all independent processing conducted by third-party technology providers.

16. COOKIES AND SIMILAR TECHNOLOGIES

Our website may use cookies and similar technologies.

Cookies may support:

Essential Functions

These may be used for:

  • security;

  • website operation;

  • session management;

  • fraud prevention;

  • network management;

  • and essential functionality.

Functional Features

These may remember visitor preferences or enable optional website functions.

Analytics

Analytics technologies may help us understand:

  • how visitors interact with the website;

  • which pages are visited;

  • general traffic patterns;

  • and website performance.

Marketing Technologies

If marketing, advertising, remarketing, social media integrations, or similar technologies are used, additional cookies or tracking tools may be deployed.

Where legally required, non-essential cookies will be subject to an appropriate consent mechanism.

Visitors may also be able to manage cookies through browser settings or available website privacy controls.

Disabling certain cookies may affect website functionality.

17. DATA MINIMIZATION

Praxis seeks to limit personal information collection to information reasonably necessary for legitimate business purposes.

This principle is especially important for International Patient Services.

For an initial patient-services inquiry, Praxis generally needs coordination information, not complete clinical documentation.

Users are encouraged to provide only the minimum information necessary for Praxis to understand their request.

18. DATA SECURITY

Praxis takes reasonable administrative, technical, and organizational measures designed to protect personal information against:

  • unauthorized access;

  • unlawful disclosure;

  • misuse;

  • accidental loss;

  • alteration;

  • destruction;

  • and other security risks.

Depending on the circumstances, safeguards may include:

  • access controls;

  • authentication measures;

  • restricted access;

  • reputable technology platforms;

  • secure business systems;

  • data minimization;

  • and cybersecurity monitoring.

However, no website, email system, telecommunications platform, cloud environment, or electronic transmission method can be guaranteed to be completely secure.

Praxis therefore cannot guarantee absolute security.

Users should avoid transmitting unnecessary sensitive information through ordinary website forms or unsecured email.

19. DATA RETENTION

Praxis retains personal information only for as long as reasonably necessary for the purpose for which it was collected or for legitimate legal, contractual, accounting, security, or business purposes.

Relevant considerations may include:

  • whether an inquiry remains active;

  • whether a client or business relationship exists;

  • contractual requirements;

  • legal obligations;

  • accounting requirements;

  • regulatory obligations;

  • security requirements;

  • dispute resolution;

  • and applicable limitation periods.

When information is no longer reasonably required, it may be:

  • deleted;

  • securely destroyed;

  • anonymized;

  • or otherwise handled in accordance with applicable law.

Backup systems may temporarily retain information until normal backup cycles replace or delete it.

20. INTERNATIONAL DATA TRANSFERS

Praxis may interact with individuals, healthcare organizations, suppliers, consultants, and technology providers located in different countries.

Personal information may therefore be:

  • transferred internationally;

  • stored outside your country;

  • processed outside your country;

  • or accessed by authorized recipients in another jurisdiction.

Data-protection laws vary between countries.

Where applicable law requires specific safeguards for international transfers, Praxis will seek to use an appropriate lawful transfer mechanism.

When you specifically request assistance involving a healthcare provider or organization in another country, information reasonably necessary to facilitate that request may need to be communicated internationally.

21. YOUR PRIVACY RIGHTS

Depending on the laws applicable to you, you may have rights including the right to:

  • request confirmation that we process your information;

  • access personal information;

  • request correction of inaccurate information;

  • request deletion;

  • request restriction of certain processing;

  • object to certain processing;

  • withdraw consent where processing relies on consent;

  • request data portability;

  • opt out of certain sales, sharing, targeted advertising, or profiling where applicable;

  • lodge a complaint with a competent privacy authority;

  • and exercise other rights available under applicable law.

These rights are not absolute.

We may decline or limit a request where permitted or required by law, including where retention is necessary to:

  • comply with legal obligations;

  • maintain transaction records;

  • protect security;

  • prevent fraud;

  • exercise contractual rights;

  • or establish, exercise, or defend legal claims.

22. EUROPEAN ECONOMIC AREA AND UNITED KINGDOM

Where the European Union General Data Protection Regulation (“GDPR”), United Kingdom GDPR, or comparable legislation applies, individuals may have additional rights.

Praxis may rely on legal bases including:

  • consent;

  • contractual necessity;

  • legitimate interests;

  • compliance with law;

  • protection of vital interests;

  • or another legally available basis.

Where special-category personal data, including certain health information, is processed, Praxis will rely on an additional lawful condition where required.

Applicable rights may include:

  • access;

  • rectification;

  • erasure;

  • restriction;

  • objection;

  • portability;

  • withdrawal of consent;

  • and the right to complain to a competent supervisory authority.

23. U.S. STATE PRIVACY RIGHTS

Residents of certain U.S. states may have additional privacy rights under state law.

Depending on the jurisdiction and whether the relevant law applies to Praxis, those rights may include:

  • access;

  • correction;

  • deletion;

  • portability;

  • opting out of certain sales;

  • opting out of certain sharing;

  • opting out of targeted advertising;

  • limiting certain processing of sensitive information;

  • opting out of certain profiling;

  • and appealing a denied privacy request.

Applicability may depend on factors including statutory thresholds, the nature of the business, and the type or amount of information processed.

Praxis will honor legally applicable privacy rights.

24. SALE, SHARING, AND TARGETED ADVERTISING

Praxis does not operate a business model based on selling personal information.

Praxis does not sell personal information for monetary compensation.

Certain laws define “sale” or “sharing” more broadly than ordinary commercial usage.

If Praxis later engages in processing that constitutes a regulated sale, sharing, or targeted-advertising activity, Praxis will provide any notices or opt-out mechanisms required by applicable law.

25. MARKETING COMMUNICATIONS

If you voluntarily subscribe to marketing or informational communications from Praxis, we may send communications to the contact information you provide.

Where an unsubscribe option is available, you may use it to stop receiving promotional communications.

You may also contact Praxis directly to request removal from marketing communications.

Unsubscribing from marketing does not necessarily prevent us from sending:

  • responses to inquiries;

  • service communications;

  • transactional notices;

  • legal notices;

  • security communications;

  • or other non-promotional messages.

26. CHILDREN AND MINORS

The general Praxis website is not directed to children under 13, and Praxis does not knowingly seek personal information directly from children under 13 through general website forms.

Where International Patient Services involves a minor, communications should generally be handled by or with the participation of:

  • a parent;

  • legal guardian;

  • authorized representative;

  • healthcare provider;

  • or another person legally permitted to act for the minor.

If Praxis learns that information was improperly collected from a child in violation of applicable law, Praxis will take reasonable steps to address the information as required.

27. INFORMATION ABOUT ANOTHER PERSON

If you provide information to Praxis about another person, such as a:

  • patient;

  • family member;

  • colleague;

  • employee;

  • healthcare professional;

  • or business representative,

you should ensure that you have appropriate authority or another lawful basis to provide the information where required.

Do not provide another person's detailed medical records or sensitive information without appropriate authorization.

Praxis may request confirmation of authority when reasonably necessary.

28. THIRD-PARTY WEBSITES

The Praxis website may include links to:

  • hospitals;

  • clinics;

  • suppliers;

  • business partners;

  • professional organizations;

  • government resources;

  • social media platforms;

  • or other websites.

Those third parties maintain their own privacy practices.

Praxis is not responsible for the independent privacy, security, or data-handling practices of third-party websites except where responsibility cannot lawfully be excluded.

You should review the privacy notices of third-party services before providing information directly to them.

29. SOCIAL MEDIA AND EXTERNAL PLATFORMS

If you communicate with Praxis through LinkedIn or another external platform, that platform may independently collect or process information regarding your interaction.

Your use of those platforms is governed by the platform's own privacy terms.

Information intentionally made public on social media may also be visible to Praxis and other parties.

30. AUTOMATED DECISION-MAKING

Praxis does not currently intend to use information collected through its general website to make decisions that produce legal or similarly significant effects on individuals solely through automated processing.

If this practice materially changes, this Privacy Policy may be updated and additional disclosures may be provided where legally required.

31. PRIVACY REQUEST VERIFICATION

Praxis may take reasonable steps to verify the identity or authority of an individual submitting a privacy request.

Verification requirements may depend on:

  • the type of request;

  • the sensitivity of the information;

  • applicable law;

  • and the information reasonably available to Praxis.

Authorized representatives may make requests where permitted by law, subject to appropriate verification.

32. SECURITY INCIDENTS AND DATA BREACHES

Praxis maintains procedures designed to respond to suspected security incidents.

If an incident involving personal information triggers a legally applicable breach-notification requirement, Praxis will seek to provide any required notifications to affected individuals, regulators, business partners, or other parties within the periods required by applicable law.

Different breach-notification laws may apply depending on the type of information and the circumstances.

33. LEGAL REQUESTS AND PROTECTION OF RIGHTS

Praxis may preserve, process, or disclose information where reasonably necessary to:

  • comply with applicable law;

  • comply with lawful court orders;

  • respond to valid governmental or regulatory requests;

  • respond to subpoenas or other lawful process;

  • investigate fraud;

  • prevent unlawful activity;

  • protect Praxis's property or systems;

  • protect individuals from harm;

  • enforce agreements;

  • or establish, exercise, or defend legal claims.

34. BUSINESS-TO-BUSINESS INFORMATION

Praxis may process professional information relating to representatives of:

  • healthcare organizations;

  • businesses;

  • suppliers;

  • manufacturers;

  • government organizations;

  • consultants;

  • prospective clients;

  • nonprofit organizations;

  • and other institutions.

Such information may be used to:

  • maintain professional relationships;

  • evaluate business opportunities;

  • administer agreements;

  • respond to inquiries;

  • conduct procurement;

  • provide services;

  • and communicate regarding legitimate business matters.

35. AGGREGATED AND DE-IDENTIFIED INFORMATION

Where legally permitted, Praxis may create or use aggregated or de-identified information that does not reasonably identify a particular individual.

Such information may be used for:

  • analytics;

  • operational planning;

  • service improvement;

  • business analysis;

  • internal research;

  • and understanding general trends.

Where required by applicable law, Praxis will maintain such information in de-identified form and will not attempt to re-identify individuals except as legally permitted.

36. CHANGES TO THIS PRIVACY POLICY

Praxis may update this Privacy Policy periodically to reflect:

  • changes in our services;

  • changes in technology;

  • changes in website functionality;

  • changes in business practices;

  • legal or regulatory developments;

  • or other operational needs.

The Last Updated date at the top of this Policy will be revised when changes are made.

Where applicable law requires additional notice or consent for material changes, Praxis will take appropriate steps.

37. CONTACT US

Questions, concerns, complaints, or requests regarding this Privacy Policy or Praxis's handling of personal information may be submitted via the contact information found on the contact page of the website.

When contacting us regarding privacy matters, please provide enough information for us to understand and respond to your request.

Please do not include unnecessary medical records, passwords, financial credentials, or other highly sensitive information in an initial privacy request.

38. MEDICAL EMERGENCIES

The Praxis website and International Patient Services are not emergency medical services.

Do not use the Praxis website, contact forms, or business email addresses to request emergency medical assistance.

If you are experiencing or believe you may be experiencing a medical emergency, contact an appropriate local emergency service or licensed healthcare provider.

39. NO EXPANSION OF REGULATORY STATUS

Nothing in this Privacy Policy is intended to:

  • establish Praxis as a healthcare provider where it is not acting as one;

  • establish Praxis as a HIPAA-covered entity where the law does not classify it as such;

  • establish Praxis as a HIPAA business associate except where a qualifying relationship actually exists;

  • convert voluntarily disclosed health-related information into PHI where HIPAA does not legally apply;

  • expand any privacy statute beyond its lawful scope;

  • create fiduciary duties not otherwise imposed by law;

  • create contractual privacy rights beyond those required by applicable law or an express written agreement;

  • or waive any lawful right, defense, limitation, or protection available to Praxis.

Where a non-waivable legal requirement conflicts with this Privacy Policy, the applicable legal requirement will control to the extent of the conflict.

40. GOVERNING PRIVACY REQUIREMENTS

Praxis seeks to comply with privacy and data-protection requirements applicable to its actual activities.

Because privacy laws differ among jurisdictions, the rights and obligations applicable to a particular interaction may depend on factors such as:

  • where the individual is located;

  • where the information is processed;

  • the type of information involved;

  • the nature of the service;

  • and the legal relationship between Praxis and other organizations involved.

Nothing in this Privacy Policy should be interpreted as voluntarily submitting Praxis to a privacy statute that would not otherwise apply.
